Forms Required to be a Crew Member

NOTE: Saline High School Athletic Department uses electronic signature for all forms common to the department. The "Common Forms" are to be completed on the Saline Area Schools website www.salineschools.org Go to: >Departments >Althletics >Participation (it takes less than 5 minutes to complete the online forms). Additionally, Saline Crew is a club sport and not part of "pay to play".

The following forms are to be completed and turned in at the mandatory parent / rower meeting each season (or as otherwise noted):

1. 2013 Rowers Info Form (updated each season)
2. Crew Team Transportation Authorization Form (update each season)
3. 2012 US Rowing Liability Release Form (update each calendar year electronic signature required online - REFER TO INSTRUCTIONS BELOW*) - This form requires an electronic signature.

4. Sports Physical Form
5. Swim Test Information and Certification Form (new rower only)
